The type of personal information I collect and process:

• Identification & contact details including client address, telephone, email address.
• GP contact details
• Emergency contact / carer / NOK details
• Past medical history and current medication used
• Notes will be made and stored to record the content of each contact

 How I get the personal information and why we have it:

The personal information I process is provided to me directly by you for one of the following reasons:

• To plan treatment and schedule appointments
• To understand your health status in relation to your sleep/ wellbeing.
• To update GP if required
• To liaise with named people if required

I occasionally also receive personal information indirectly, from the following sources in the following scenarios:

• GP or named people in rare cases as necessary to support treatment.

I use the information that you have given us in order to ensure treatment is appropriate to your health and circumstances.

I may share this information with GP or named person if I have concerns or you require other assessments.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are:

How I store your personal information: 

Your information is securely stored using password protected cloud-based files.

 I keep contact details and notes relating to your treatment for seven years. I will then dispose your information by deleting all files relating to our communication and contacts.

Your right of access - You have the right to ask for copies of your personal information. 

Your right to rectification - You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. 

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances. 

Your right to restriction of processing - You have the right to ask me to restrict the processing of your personal information in certain circumstances. 

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
